The People's Joker builds its entire narrative around the protagonist's gender dysphoria and medical transition as the central conflict and resolution.
The story follows Joker the Harlequin from childhood questioning of being born in the wrong body, through conversion-therapy-style suppression with Smylex, to affirmation via a T4T romance with Mr. J (a reimagined trans man Jason Todd) and immersion in a vat of estrogen. This premise subordinates the Batman universe parody to progressive identity exploration, turning the film into a vehicle for ideological messaging rather than entertainment. Creator Vera Drew explicitly frames the project as autobiographical mythologizing of her own trans coming-out, abusive relationship, and grievances against comedy industry norms and family transphobia.
Established DC characters receive deliberate transgender or nonbinary reinterpretations, including nonbinary Poison Ivy with they/them pronouns and Mx. Mxyzptlk, while Batman is recast as a fascistic, closeted gay abuser heading a dystopian regime that enforces patriarchal and anti-queer control.
