The premise draws from traditional rape-revenge and body-horror conventions rather than identity politics or systemic-oppression narratives. Directors Jen and Sylvia Soska have described their interest in third-wave feminism and female ownership of sexuality, which informs the female protagonist's arc and critique of male objectification in medicine, but these remain secondary to the gore-driven plot and character descent.

Casting features an original white female lead with no source-material alterations or forced diversity mandates. Audience and critic reception highlights mixed views on its feminist elements without widespread labeling as activist messaging or significant backlash tied to contemporary social justice themes.
